As part of the assessment, the nurse should evaluate the impact of the service user’s mental state on the risk of violence and aggression, and record any risk in the notes. Encourage service users to recognise their own triggers and early warning signs of violence and aggression and other vulnerabilities, and to discuss and negotiate their wishes should they become agitated. Include this information in care plans and advance statements and give a copy to the service user. Consider offering service users with a history of violence or aggression psychological help to develop greater self‑control and techniques for self‑soothing. If a service user has not made any advance decisions or statements about the use of restrictive interventions, encourage them to do so as soon as possible (for example, during admission to an inpatient psychiatric unit). Ensure that service users understand the main side‑effect profiles of the medications recommended in this guideline for rapid tranquillisation (see recommendation 1.4.37) so that they can make an informed choice.

Alcohol and Teens

It is difficult to evaluate the independent influences of the perpetrators’ and victims’ drinking because they are confounded in most sexual assaults. Researchers need to acknowledge this problem when they are unable to separately consider the effects of each individual’s alcohol consumption (Martin & Bachman, 1998). Research is needed with very large samples so that the unique effects of perpetrators drinking alone, victims drinking alone, and perpetrators and victims drinking together can be determined. Expectancies are often self-fulfilling; people tend to process ambiguous information in such a way that it confirms their initial hypothesis (Snyder & Stukas, 1999). If a man who is on his fifth date with a woman is expecting the evening to end with sexual intercourse, then he is likely to interpret any smile or touch as a sexual sign (Roche & Ramsbey, 1993).

Spousal abuse is a concern among adults, not only for the physical toll it can take but also for the emotional trauma involved. Although both genders can commit spousal abuse, men are responsible for a majority of incidents. For instance, 7.7% of all women in the United States reported being the victim of sexual violence, and about 22% of women were physically assaulted by a male partner at some point in their lives (Krug et al., 2002). Because nurses are the primary caregivers to these victims, they are active in helping them recover physically and emotionally. There is a subset of aggressive behavior in adolescents, however, that appears in adolescence and then disappears in early adulthood. • Undertake manual restraint with extra care if the service user is physically unwell, disabled, pregnant or obese.

Parents who have substance use drug addiction problems will likely have their own affect dysregulation that may have preceded or resulted from their substance use. Consequently, development of healthy affect regulation will be difficult for children and adolescents to achieve. Children may present to a social worker in direct practice at community mental health center or a school setting.
